Service agreement

This Referral Agreement (“Agreement”) is made, entered into and effective as of (the “Effective Date”) by and between (“Agent/Broker”) licensed in the state of (“Agent/Broker’s State(s)/Province(s)”), and Zaivex LLC (its affiliates, subsidiaries, and authorized payment processors Care Business Consulting Inc), collectively referred to as “Company,” and the Agent/Broker, a Florida LLC with a mailbox addressed at 8051 N. Tamiami Trail STE E6, Sarasota, Florida.

Agent/Broker and Zaivex LLC are each a “Party” and are collectively referred to as the “Parties” to this Agreement.

Zaivex LLC connects homeowners directly to your business through live consumer-initiated inbound phone calls. You only pay for real, qualified opportunities. No hidden charges.

Performance-based billing

You’re only charged for qualified calls that meet your criteria:

• The caller is a homeowner.
• The caller is located within your approved service area.
• The caller is seeking services your company provides.

Transparent and fair

No questions asked for scrubbed calls — if a call clearly doesn’t qualify, we remove it.

Any calls above 240 seconds are still billable, provided they meet the above qualification criteria. Calls that have been paid for cannot be marked billable again until after 60 days has passed. After that, if they call again and convert, they will be a billable lead.

Flexible commitment

Cancel anytime — there’s no long-term contract or volume minimums.

Payment terms

Automatically charged upon the completion of the call that is above 240 seconds.

Call quality standards

The default converted call duration is 240 seconds. Calls reaching or exceeding this threshold are automatically billable unless scrubbed for clear disqualification. Zaivex continually monitors call quality, caller intent, and location matching to ensure your spend generates real customer opportunities.

Billing method

Client authorizes Zaivex LLC to securely process the monthly payments through merchant via saved card on file upon signature.

Cost per call

Leads from our network will be generated through all available marketing methods and billed upon the completion of the call (equal to or above 240 seconds) to the credit card on file. Cost per call is based on the service type and coverage area. Calls are considered billable once they reach the default 240-second duration threshold.
